Sham ko halka bukhar (99-101°F): Subah theek, sham ko bukhar, thakan. Yeh TB ki khas alamat hai.
Wazan kam hona aur bhok khatam: 1 mahine me 3-5 kg wazan kam, kapray dheelay.
Raat ko pasina: Neend me itna pasina ke kapray badalne parain.
Balgham me khoon (Hemoptysis): Khoon ki chittein ya khoon ki khansi — foran doctor ko dikhayein.

Step-wise Testing at IRC:

Step 1 - Digital Chest X-ray (PA + Lateral): First screening. Infiltration, cavity, pleural effusion dekhta hai. 10 min me report.

Step 2 - Sputum GeneXpert MTB/RIF (Gold Standard): WHO recommended. Subah ka gehra balgham ka sample — 2 ghante me result + Rifampicin resistance (MDR) batata hai. Sensitivity 95% pulmonary TB me.

Step 3 - Sputum AFB Smear & Culture: Agar GeneXpert negative lekin shak zyada ho to culture gold standard hai — 2-6 haftay me result. Drug-susceptibility testing (DST) is se hota hai.

Step 4 - Bronchoscopy with BAL GeneXpert (IRC me available): Jab balgham na aa raha ho, X-ray doubtful ho, ya tumor vs TB ka farq karna ho. Bronchoscopy se andar se wash le kar GeneXpert karte hain — accuracy 85%. General anesthesia nahi, local spray.

Step 5 - Pleural Fluid / Lymph Node FNAC: Pleural TB ke liye fluid ADA, protein, GeneXpert. Gardan ki ghilti ke liye FNAC + GeneXpert.

Step 6 - Blood Tests: CBC, ESR, CRP, LFT, KFT, RBS, HIV, HbA1c before treatment — dawa liver par asar karti hai to baseline zaroori.

Tip: Subah uthte hi pehla gehra balgham ka sample sab se behtar. Mouth wash karke, gehra saans le kar balgham nikalein, thook nahi. 2 samples ideal — one spot (clinic me), one early morning (ghar se).

6. TB Ka Ilaj - DOTS 6 Months Protocol

7. MDR & XDR TB - Resistant TB Ka Ilaj

MDR-TB (Multi-Drug Resistant): Rifampicin + Isoniazid dono se resistant. Wajah: adhi dawai chorna, dose miss karna, galat regimen.

GeneXpert RIF resistance batata hai → LPA (Line Probe Assay) + Culture DST se confirm
Ilaj 9-12 months, second-line drugs (Bedaquiline, Linezolid, Moxifloxacin, Clofazimine) — mehenga aur side effects zyada
Cure rate 60-70%, regular follow-up zaroori

XDR-TB: MDR + Fluoroquinolone + second-line injectable se bhi resistant — aur mushkil ilaj.

Prevention: Pehli dafa hi DOTS poora karein, koi dawai miss na karein, doctor ke mashwaray ke bagair dawai na chorhein.

8. Dawaon Ke Side Effects & Liver Test

Peshab narangi (orange): Rifampicin se — normal hai
Matli, bhok kam: Pyrazinamide se — khane ke baad lein, doctor ko batayein
Jorhon me dard: Pyrazinamide se uric acid barhta hai — pani zyada piyein
Nazar dhundli: Ethambutol se — agar nazar kam lage to foran doctor ko batayein, color vision test karayein
Jaundice (peeli rangat): Isoniazid, Rifampicin, Pyrazinamide liver par asar kar sakti hain — agar aankhein peeli, bhok bilkul khatam, ulti aaye to foran LFT karayein aur doctor ko dikhayein. Alcohol bilkul mana.

9. TB Me Diet & Nutrition Plan

High Protein + High Calorie Diet — Wazan kam na hone dein:

Anda roz 1-2, chicken, fish, daal, lobia — protein lungs repair ke liye
Doodh, dahi, lassi — calcium aur protein
Roti, chawal, aalu — energy ke liye
Mausami phal: kela, amrood, orange (Vitamin C)
Sabzian: palak, gajar, bhindi — vitamins
Pani 8-10 glass, ORS agar paseena zyada

Avoid: Smoking, naswar, ghutka, sharab (liver damage), bahar ka khana, zyada mirch masala, cold drinks. Chaye kam piyein.
